Before you use one

Part 6 is fixed. The certification is the part that makes a PROOF form worth something to the person receiving it. A form with an altered Part 6 is not a PROOF form. Everything outside Part 6 is data: fill it in, and a blank is not an answer.

Fill every field. Where a report was not required, record the reason. A blank line and “not required” are not the same thing. The thirteen rules take one minute to read and explain why each of those matters.

The Word versions are there to be filled in, not redrafted. Editable is the point, so you can type into the tables and add rows where a party list runs long. It is not an invitation to rewrite the certification. That is rule 13, and it is the whole reason a standard is worth having.
